JOB DESCRIPTION
Summary:
The On-Call Registered Nurse plans, organizes and directs the hospice care of patients in the field after hours in conjunction with the Hospice Interdisciplinary Team.
The RN provides direct patient care with an emphasis on palliative care, provides education and support to families and caregivers The RN must be capable of compassionate communication with patients and their families, and must have keen observation skills, high ethical standards, and knowledge of when to alert other members of the IDT about changes in the patient's condition.
A hospice nurse must be resilient and sympathetic, and should have emotional and physical stability to deal with the challenges of severe illness and death.
Ability to work independently and with autonomy.
Essential Job Functions/
Responsibilities:
1.
Responsible for responding to and evaluating the emergent needs of patients and families after hours implementing necessary interventions to palliate symptoms.
Ensures all arrangements for equipment and other necessary items and services are available.
2.
Initiates and provides direct patient care and appropriate nursing interventions and procedures, assists with activities of daily activities, and administers medications and treatments as prescribed by the attending physician or medical director.
3.
Visits and evaluates patients who have been referred to Talbot Hospice to determine eligibility for enrollment in hospice benefit and consults with Hospice Medical Director/Associate Directors for hospice appropriateness.
4.
Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
Provides a complete physical assessment and history of current and previous illness.
Completes all admission tasks per Talbot Hospice protocol.
5.
Initiates and develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
Includes the patient and the family in the planning process.
Communicates with IDT members to coordinate the care plan and attends IDT meetings.
6.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient and family/caregiver as appropriate per assessment and plan.
7.
Completes, maintains and submits accurate and relevant clinical notes in the electronic medical record regarding patient's condition and care given.
8.
Supervises ancillary personnel and delegates responsibilities when required.
Supports Hospice House caregivers.
Additional Duties:
1.
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
2.
Fulfills the obligation of requested and/or accepted case assignments.
3.
Actively participates in quality assessment performance improvement teams and activities.
Position
Qualifications:
1.
Graduate of an accredited school of nursing.
One to two years of recent Med-Surg experience preferred.
2.
Current licensure in Maryland.
3.
Bachelor's degree, with hospice care experience a plus; expected to achieve CHPN status by end of third year.
4.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills.
5.
Must be a licensed driver with an automobile that is insured in accordance with policy requirements and is in good working order.
